WHILE ASCENDING THE 4 NORTH GRADE WITH A TRIP OF LOADS THE OVERCURRENT DEVICES DEENERGIZED ON THE REAR LOCOMOTIVE CAUSING THE TRIP TO STOP ON THE GRADE AND CHANGE DIRECTION OF TRAVEL. THE FRON T MOTORMAN APPARENTLY JUMPED OR FELL FROM THE DECK OF THE LOCOMOTIVE AND STRUCK HIS HEAD ON AN UNKNOWN OBJECT

    At the time of filing it is believed the miner was riding on the back of a railcar when the railcar derailed and pinned the miner between the railcar and a nearby structure. The miner was transported to a medical facility where EE died the following day.
