EMPL SCALING DOWN LOOSE MATERIAL IN STEEL REHAB PROCESS. LOOSE SLAB 2\ X 4\ X 6\ SUDDENLY FELL FROM HIS OUTWARD SIDE HITTING SCALING BAR WHICH THREW HIM IN A FORWARD POSITION.THE TAIL END OF T HE SLAB KNOCKED I.NAME OFF BOLT CAR HITTING HIM IN THE HEAD,SHOULDERS AND LEG.

    Operator was conducting remoting activities in an area known as the 2025 area when a large rock contacted the cab. Operator was found in the cab out past the designated no-go line and sustained fatal injuries.
