I.NAME & I.NAME WERE WORKING ON TOP OF A BULKHEAD WHICH COVERED THE ORE PASS. I.NAME WAS CUTTING 3"X12" PLANK & I.NAME WAS CUTTING STEEL BEAM WITH A TORCH. I.NAME ASKED I.NAME TO GET A CHIPP ING HAMMER, I.NAME WALKED TOWARDS THE TOOL BOX STOPPED ON A PLANK- OR PLANKS- NOT SUPPORTED ON THE ENDS & FELL DOWN THE RISE.
