A RAISE LINER W9IGHING 60,000 LBS WAS BEING LOWERED IN 4900-5100 ORE PASS.THE LINER WAS SUSPENDED ON 2 WIRE ROPES,EACH 1'' DIA.,GRIPPED BY 4 LUCKER GLAND TYPE DEVICES.SUSPENSION SYSTEM FAILED AND I.NAME WAS3STRUCK BY ONE OF THE WIRE ROPE STORAGE REELS AND WAS FATALLY INJURED.
