AT 2:25 PM A CR9W WORKING AT THE JOB SITE SET OFF A BLAST WHILE I.NAME WAS SEATED IN HIS PICKUP TRUCK ABOUT 300 FEET AWAY, DOWN OVER A LARGE EMBANKMENT FROM THE BLAST AREA. AS THE CHARGE WAS D ETONATED, I.NAM9 LOOKED UP SAW DEBRIS FALLING TOWARD HIM AND CALLED OUT TO WARN OTHERS NEAR HIM. A LARGE CHUNK OF CLAY-ROCK STRUCK THE ROOF OF THE TRUCKS CAB, CAVING IT IN, KILLING I.NAME INS

    The detonator had a dead battery. In order to set off the shot, employee brought up a loader. The loader battery was used to set the shot off. Employee was unable to be inside the shelter provided for blasting.
