THE VICTIM WAS 1USHING SAND INTO THE FEEDER/HOPPER WITH A SHOVEL AND FELL INTO A CONE SHAPED HOLE. HE WAS ENGULFED BY MATERIAL THAT FELL IN BEHIND HIM. THE VICTIM WAS NOT WEARING A SAFETY BELT OR A TAG LINE 4HICH WAS PROVIDED NOR WAS HE USING THE 9 FOOT BAR DESIGNED AND PROVIDED TO KNOCK DOWN THE HANGUP.
