I.NAME WAS REMO5ING A 10' SECTION OF BOOM FROM A 108 LINK BELT CRANE. I.NAME FAILED TO BLOCK & SECURE THE BOOM BEFORE POSITIONING HIMSELF UNDER THE BOOM TO REMOVE THE LAST BOTTOM PIN. WHEN THE LAST PIN W5S REMOVED THE BOOM COLLAPSED PINNING I.NAME BETWEEN THE BOOM & GROUND.
