EMPLOYEE CRAWLE8 UNDER BEND PULLEY GUARD TO RETRIEVE A HAMMER AND ON H IS WAY OUT HE BECAME ENTANGLED BETWEEN THE CONVEYOR BELT AND PULLEY. HE WAS PULLED THRU AND HE DROPPED 34 FT TO THE LEVEL BELOW. AL5 CRUSHING INJURIES WERE SUSTAINED DURING THE TRIP THRU THE PULLEY
