I,NAME WAS FATA5LY INJURED WHEN AN UNPLANNED SLOPE FAILURE OCCURRED. SEVERAL THOUSAND TONS OF LARGE BOULDERS AND LOOSE MATERIAL FROM APPROX. 70 FT HIGH PIT HIGHWALL FAILED, BURYING AND CRUSHIN G THE TRACK DRI5L OPERATOR.
