EMPLOYEE WAS DRIVING A 769 CATERPILLAR ROCK TRUCK HAULING REFUSE TO THE SLURRY POND WHEN HE BACKED UP TO DUMP HIS LOAD THE TRUCK WENT OVER THE EMBANKMENT FLIPPED OVER UPSIDE DOWN OUT IN THE SL URRY POND THE CAB SANK IN THE SLURRY TRAPPING THE DRIVER
