EMPL WORKING ON STRUCTURAL STEEL SECTION OF ELEVATED WATER TOWER -NORMAL DUTIES AS A WELDER.WELDING ON 2ND OF 2 SECTIONS.EMPL WAS WEARING SAFETY BELT WITH 2 LANYARDS ATTACHED AND WAS REQUIRED TO BE TIED OFF AT ALL TIMES WHEN WORKING.EMPL FELL 93' WITH 1 LANYARD ATTACHED TO BELT AND 1 ATTACHED TO HAND LINE.
