UNLOADING PIPE FROM TRUCK. TRYING TO LEVERAGE LENGTH OF PIPE OFF THE TRAILER ONTO THE FORKLIFT. ALMOST HAD PIPE ON FORKLIFT WHEN IT BEGAN TO ROLL BACK TO DECEASED.
