I NAME WAS GUIDING A GRADELL INTO PLACE TO BREAK UP CARBON BLOCKS STANDING BY THE DRIVERS DOOR I NAME TOLD THE TURRET OPERATOR OK SWING THE BOOM AROUND TO CHECK THE CLEARANCE UNKNOWN TO THE TU RRET OPERATOR & VEHICLE DRIVER I NAME APPARANTLYTHEN MOVED REARWARD & WAS PINNED AGAINST THE WALL BY THE REAR OF THE TURRET WHY I NAME MOVED FROM HIS SAFE POSITION IS UNKNOWN

    EE and another employee were pulling the engine and removing nose cone on the radiator. EE was on the bottom under the 992g wheel loader. The other employee was on top of the machine. EE was taking the radiator nose off, accidentally took bolts out of counter weights and it fell on EE. Resulting in a fatality.
