I.NAME -SUB CONTRACTOR-FAILED TO FOLLOW SAFETY PRECAUTIONS PERTAINING TO HANDLING OF EXPLOSIVES,FAILURE TO REMOVE PERSONNEL FROM BLAST ZONE RESULTED IN DEATH OF 1 OF THE TWO PERSONS LEFT IN TH E BLAST ZONE,SERIOUS INJURY TO THE OTHER.

    The detonator had a dead battery. In order to set off the shot, employee brought up a loader. The loader battery was used to set the shot off. Employee was unable to be inside the shelter provided for blasting.
