EE WAS WORKING SWING SHIFT (3:PM TO 11:30PM) PERFORMING MAITENANCE WORK ON THE DRAG SCRAPER LOCATED ON THE 4TH FLOOR OF THE TRUCK DUMP. AT 9:58PM, HE NOTICED SOME SMOLDERING COAL. AT 10:01 P M WITH WATER HOSE IN HAND HE STARTED HOSING DOWN WITH WATER THE SMOLDERING COAL. CAUSING A FLASH BACK. RESULTING IN FIRST AND SECOND DEGREE BURNS TO HIS RIGHT HAND AND FOREARM.

    988 Loader caught fire, was unable to be extinguished by site personnel, 911 was called. Two fire crews arrived on site, one fireman approached the loader by the rear tire. The plant manager was yelling at them to get away from the tire. It exploded killing the fireman by the tire.

    The IE was hauling overburden in a Cat 793 end dump to a fill area. The front area on the end dump caught fire. The IE parked the truck on the fill and exited the end dump.
