AN ON GOING CO INVESTIGATION IS BEING CONDUCTED TO IDENTIFY THE CONTRIBUTING FACTORS TO THE ACCIDENT PRELIMINARY FINDINGS INDICATE THAT THE ACCIDENT RESULT FROM A SUDDEN AND UNEXPECTED RELEASE OF SLURRED MATERIAL FROM THE TUBE 60 INCH 300 FT BORHOLE INTO THE CHUTE AND BIN BELOW.
