FATALLY INJURED EMPLOYEE WAS ELECTROCUTED WHILE WORKING ON ENERGIZED HIGH VOLTAGE CABLE.THE DISCONNECT SWITCHES "60 FT OUTBY" HAD NOT BEEN PULLED AND LOCKED OUT AS REQUIRED.
