A "AA" TRUCK DRIVER, AFTER INITIALLY STRIKING A WELL ESTABLISHED BERM WHILE BACKING, THEN PULLED FORWARD APPROX. 25'. UPON HIS SECOND ATTEMPT IN BACKING, AT A SPEED GREATER THAN NORMAL, HE WEN T THROUGH SAID BERM, DOWN THE DUMP, RESUTLING IN FATAL INJURY.
