EMPLOYEE WAS WORKING ON THE ROOF OF T-4B TANK WHEN COMPRESSION RING FAILED WHICH CAUSED THE EMPLOYEE TO FALL TO THE BOTTOM OF THE TANK, SUFFERING APPARENTLY FATAL INJURIES.
