Tipple/Top Lander/Skip Dumper Operator fatality
HE WAS INJURED AS THE RESULT OF BECOMING ENTRAPPED BY SLIDING MATERIAL WHILE WORKING INSIDE A METAL BIN (ENCLOSED). THEY WERE ATTEMPTING TO LOAD A TRUCK FROM BELOW THE MATERIAL ATTACHED TO THE WALLS AROUND THEM (3 OF 4 WALLS HAD MATERIAL ATTACHED). THE SLIDING MATERIAL BURIED HIM COMPLETELY AND ANOTHER EMPLOYEE TO HIS CHIN.

Employee was removing an electrical motor using a chain hoist suspended from a cross brace when the cross brace became unsecured and struck the employee.

The jack kicked out and employee was struck in the head by an unknown object and knocked him into the bed of the truck where he struck his head on an unknown object. Employee received head trauma.
