EMPLOYEE ATTEMPTED REMOVING A 24"X24" MAN HOLE COVER AT THE TOP OF A 27 FT. HIGH LIME SILO, THE SILO WAS UNDER PRESSURE, EMPLOYEE WAS USING A 16 OZ HAMMER TO LOOSEN THE COVER, DO TO THE EXTREN E PRESSURE IN THE SILO THE COVER CAME OFF STRIKING THE EMPLOYEE IN THE DEAD.
