EE WAS OPERATING A SHUTTLE CAR AND RAN THE SHUTTLE CAR IN THE COAL RIB CAUSING HIM TO BE PINNED BETWEEN THE COAL RIB AND SHUTTLE CAR THIS ACCIDENT WAS FATAL TO HIM RESULTING IN DEATH.
