VICTIM WAS BETWEEN RAISED DUMP BOX AND TRUCK FRAME. HE DID NOT ENGAGE THE PROVIDED MECHANISM TO KEEP BOX FROM COMING DOWN. BY CAME DOWN AND CRUSHED VICTIM. VICTIM DIED FROM ACCIDENT INSTRANTNA IOUSLY.
