55061S AIRING A TIRE ON A DISABLED LOADER BEING PREPARED FOR REPAIRS AND EVALUATION. THE SIDE WALL BLEW OUT WITH SUCH A FORCE-THROWING EE BACK APPROX. 10 FEET INTO A STONE PILE, CAUSING A CRUS 89570BLOW TO THE CHEST CAVITY RESULTING IN IRREVERSIBLE TRAUMA AND DEATH.

    An employee was attempting to clean out a previously blasted vertical drill hole with an air hose. A sudden release of pressure from an unknown source inside of the hole forced material to be ejected from the hole and struck the employee. The employee passed away from the incident.
