AT APPROX. 10:40 THEY WERE COMPLETING PINNING AND INSTALLING LINE CURTAIN IN THE FACE OF #3 RIGHT ENTRY. I.NAME WAS PUSHING UP LOOSE COAL WITH DIESEL SCOOP IN THE FACE OF #4 RIGHT ENTRY. ON CO MPLETING PUSHING UP IN #4 RIGHT, I.NAME BACKED HIS SCOOP UP TO THE OUTBY CROSSCUT IN #3 AND #4 AND TRAMMED ACROSS TO THE #2 RT OPEN LAST CROSS CUT. HE TRAMMED HIS SCOOP INTO THE SAME CROSSCUT

    EE and another employee were pulling the engine and removing nose cone on the radiator. EE was on the bottom under the 992g wheel loader. The other employee was on top of the machine. EE was taking the radiator nose off, accidentally took bolts out of counter weights and it fell on EE. Resulting in a fatality.
