EMPLOYEE RESPONDED TO BUILDING FIRE AT THE MILL REFINERY FACILITY, WITH EMERGENCY RESPONSE TEAM, IN THE COURSE OF FIGHTING THE FIRE, EMPLOYEE WAS FOUND DOWN ON THE FLOOR IN FULL FIRE GEAR AND SCBA. EMPLOYEE WAS TRANSPORTED BY AMBULANCE TO ELKO GENERAL HOSPITAL WHER HE WAS PRONOUNCED DEAD.

    988 Loader caught fire, was unable to be extinguished by site personnel, 911 was called. Two fire crews arrived on site, one fireman approached the loader by the rear tire. The plant manager was yelling at them to get away from the tire. It exploded killing the fireman by the tire.

    The IE was hauling overburden in a Cat 793 end dump to a fill area. The front area on the end dump caught fire. The IE parked the truck on the fill and exited the end dump.
