DECEASED,WELDER AND (NAME),LEAD MAN WERE WORKING IN THE TUB REPLACING RACK SEGMENT BOLTS,THE POWER TO THE DRAGLINE WAS DE-ENERGIZED AT THE SUBSTATION.OTHER EMPLOYEE UNLOCKED THE DOOR ON THE TR AILING CABLE TERMINATION ROOM TO INSPECT IT.AS WORK WAS NEARLY COMPLETED EMPLOYEE ASKED ELECTRICIAN TO RE-ENERGIZE THE POWER.AS EMPLOYEE WAS HAULING EQUIPMENT OUT OF THE DRAGLINE THE LIGHTS W
