THE EE WAS LOADING ANFO INTO AN ON SITE DELIVERY TRUCK FROM A VERTICAL STORAGE TANK. THE MATERIAL HAD HUNG UP THEN SUDDENLY CAME LOOSE. THE SUDDEN IMPACT SHEARED 20 STRUCTURAL SUPPORT BOLTS CA USING A LOWER PORTION OF THE TANK AND MATERIAL TO CRASH THRU A DECK CRUSHING AND PINNING THE EE BETWEEN THE DECK SUPPORT BEAMS AND THE HANDRAIL.
