EMPLOYEE WAS USING A HOIST TO GET A PIPE DISTRIBUTION BOX UNDER A WASHER IN A COAL PREPARATION PLANT.HE WAS STANDING ON A VIBRATING SCREEN WHEN THE BOX SWUNG AROUND AND KNOCKED HIS LEG OUT FRO M UNDER HIM.HE FELL INTO THE SCREEN AND DOWN A CHUTE TO THE NEXT FLOOR.
