AT APPROX. 1245M ON OCTOBER 29, 1996, EE WAS OPERATING A 50 TON EUCLID HAULAGE TRUCK TRANSPORTING REFUSE FROM THE PREP PLANT TO THE REFUSE PILE. AFTER DUMPING HIS LOAD OF REFUSE MATERIAL THE O PERATOR DIDN'T LOWER THE TRUCK BED. THEREFORE, APPROX. 1.6 MILES FROM THE REFUSE PILE THE TRUCK BED STRUCK THE OVERLAND CONVEYOR BELT XOVER AT THE TRANSFER STATION #1 CAUSING THE FATAL INJURY
