Investigator narrative
A 5/8" WIRE ROPE CABLE WAS BEING WED THROUGH A SHIEVE WHEEL ATTACHED TO AN ANCHOR PIN TO PULL THE STAGELOADER CONVEYOR CHAIN. WHEN THE DECEASED INDIVIDUAL WALKED INTO THE AREA, THE ANCHOR PIN FAILED CAUSING THE FATAL INJURY.
