THE VICTIM RECEIVED FATAL CRUSHING INJURIES WHILE WORKING ON A STAMLER COAL FEEDER BREAKER. THE EMPLOYEE FAILED TO LOCK AND TAG OUT THE CIRCUIT. HE WAS A CERTIFIED ELECTRICIAN AND WAS WELL AWA RE OF THE REQUIREMENTS CONCERNING LOCK OUT/TAG OUT PROCEDURES.
