A DELIVERY TRUCK DRIVER WAS FATALLY INJURED WHILE UNLOADING A LOAD OF 18" BY 50' PVC PIPE FROM A FLATBED TRAILER. WHEN HE CUT A ROPE SECURING THE PIPE ON THE TRAILER, THE PIPE ROLLED OFF AND C RUSHED HIM.

    The truck owner was replacing a bolt in a pony axle. The owner had a bottle jack under one axle tire and a wooden block under the axle tire beside it.
