EMPLOYEE WAS SERVICING HEAD PULLEY GEAR BOX ON RADIUS ARM BELT IN AREA ABOVE SAND PILE #4 AFTER PROPERLY. LOCKING OUT BELT AND ENERGY SOURCE. PROEPRLY GUARDED PLATFORM WAS PROVIDED AND USED. NOT KNOWN AS TO WHY OR HOW EMPLOYEE LEFT PLATFORM. EMPLOYEE WAS LATER FOUND AND RESCUED FROM PRODUCT CHUTE BENEATH SAND PILE #4.
