TECHNICIAN A HYDRAULIC LINE FROM VALVE BANK ON A 990 WHEEL LOADER. THIS ACTION RELIEVED HYDRAULIC OIL FROM CYLINDERS. THIS WORK WAS PERFORMED IN CLOSE WORKING AREA UNDER LOADER FRAME. THE LOAD ER FRAME THEN MOVED DOWNWARD PINNING AND SUBSEQUENTLY CRUSHING TECHNICIAN BETWEEN LOAD FRAME AND MAIN BODY OF THE 990.

    At the time of filing it is believed the miner was riding on the back of a railcar when the railcar derailed and pinned the miner between the railcar and a nearby structure. The miner was transported to a medical facility where EE died the following day.
