EMPLOYEE APPARENTLY ATTEMPTED TO EXIT MACHINE TO THE RIGHT UNDER THE BOOM. WHILE EXITING, THE EMPLOYEE'S HIP ENGAGED THE BOOM LOWER CONTROL, CAUSING THE BOOM TO LOWER AND CRUSH EMPLOYEE'S TORS O, CAUSING FATAL INTERNAL INJURIES.
