IN THE PROCESS OF CLEANING THE SILO, CONTRACTOR'S SUPERVISOR ENTERED THE SILO. A LEDGE OF HARDENED CEMENT ON WHICH HE WAS STANDING GAVE WAY CAUSING HIM TO FALL TO THE BOTTOM OF THE SILO. A RES ULTING AVALANCHE OF CEMENT CASCADED DOWN UPON HIM AND BURIED HIM.
