Over-exertion in lifting objects

On 1/4/17, Employee was shoveling and said EE injured EE's shoulder. EE did not report the injury until 1/11/17. EE was taken to the doctor and was diagnosed with a left shoulder strain and released that day for light duty. EE returned for a check up on 1/25/17 and was told that it may be a torn rotator cuff. MRI to follow.

Fall to the walkway or working surface

At the end of his shift, Employee was stepping out of the shower when he lost his balance by slipping on a sheet of cardboard injuring his head on the concrete floor. He incurred a laceration behind his right ear on his head that required staples to close the wound.

Over-exertion in lifting objects

The employee was lifting a 100# bag and lost his grip on the bag with his right hand and tried to correct it with his left hand. The weight of the bag on his left hand caused him to fracture his pinky finger bone on his left hand.

Caught in, under or between running or meshing objects

The trained worker was washing out the mixer. He placed his hand in the mixer contrary to his training to brace himself to wash the mixer. In the act of moving the water hose, the hose hit safety switch. This turned on the mixer & pulled his left hand into the mixer. He broke bones in is left hand.

Over-exertion in wielding or throwing objects

The worker was loosening a nut at the slide gate on a hopper. The nut would not loosen at first, so the worker hit the wrench with is left hand to break the nut loose. When he hit the wrench a sharp pain went up his arm, which caused his lower left forearm tendon to become inflamed.

Caught in, under or between a moving and a stationary object

While hand loading a vacuum funnel on to a pallet, the worker lost control of the load and tried to save it. His middle finger on his left hand was smash between two funnels. He was taken to the doctor to get his finger check and it was determined that he had a broken middle finger.

Inhalation of radiations, caustics, toxic and noxious substances

8:30 AM 5/14/13 Received fax from employee's doctor stating he had silicosis. Told employee we would not let him work in plant until further diagnosis. Told him we would continue to pay him his normal salary while he was off until we could get Workers Compensation started.

Struck by falling object

EE working under mixer in mudroom cleaning door of mixer when chunk of tar fell and got into eye. EE washed eye out but still had irritation. Went to E.R. was told to see eye dr. 9/26.

Employee was breaking bags and pulled on bag straining muscle in right arm.

Worker was coming down 6 ft step ladder and tripped and fell onto concrete floor injuring shoulder and ribs.

At 9:45am ee reported he saw another ee fall to ground as if passed out. EE found other ee lying on ground by bulk sack machine complaining of shortness of breath. 911 call made, when ENT arrived they check blood pressure & sugar level. Sugar level low so they administered sugar through IV seem to become alert immediately. Transported to hospital.

Employee was transferring sodium silicate from 55 gallon drum to mixer. When opening drum, sodium silicate splashed into his eyes. Employee was not wearing safety glasses. Employee's eyes were flushed out with water at eyewash station & then he wash carried to emergency room.

EE was stacking taphole blocks when block slipped & jammed thumb. EE con't to work until 3/5/07. Thumb was bothered him, EE went to Dr and Dr diagnosed closed fracture. Give meds and brace. EE cont' to work until 3/30/07 when outpatient surgery was scheduled. EE had surgery 4/2/07. RTW date unk at this time.

A cylinder was hung so employee & his co-worker could pick it up with a forklift. EE's index finger on his left hand was caught between cylinder & frame.

Fall from scaffolds, walkways, platforms

Employee was going to mixer platform to begin running mixer. Employee tripped or apparently passed out and fell over railing. Employee fell approximately 10 ft to concrete floor. Cardboard box partially broke fall. Employee said he remembers tripping and falling, nothing much after that.

EMPLOYEE HAD BEEN CLEANING OUT CLAY CRUSHING MACHINE AND WAS STANDING ON THE PLATFORM AROUND THE MACHINE. HE BEGAN WALKING AND APPARENTLY MADE A MIS-STEP WITH HIS LEFT FOOT WITH RESULTING SPRAIN OF LEFT ANKLE.

EMPLOYEE WAS CALLED TO ""MUD ROOM"" TO REPAIR V BELT THAT HAD COME OFF THE HEAD ROLLER PULLEY ON THE DISCHARGE BELT FROM THE MIXER. NO ONE WAS AWARE HE CAME IN AND STARTED REPAIRS. HE BEGAN TO R EPAIR V BELT WHEN MIXER OPERATOR ENGAGED BELT TRAPPING HIS HAND BETWEEN PULLEY AND BELT. THE DOCTOR TRIED TO SAVE HIS FINGER BUT LATER AMPUTATEDTHE RIGHT INDEX TO 1ST JOINT.

Fall from machine

EE WAS PERFORMING MAINTENANCE WORK ON PIT BULLDOZER. AS HE WAS GETTING OFF THE DOZER HE TRIPPED AND FELL STRIKING THE GROUND.

EMPLOYEE ENTERED AREA TO TURN ON EXHAUST FAN IN ORDER TO EXHAUST THE FUMES WHERE AN OVEN WAS HEATING FUMES.

EMPLOYEE BROUGHT DOCTOR'S STATEMENT ON 9/10/01 AT 9:00 AM INDICATING HE HAD EMPHYSEMA AND SILICOSIS. HE HAS NOT WORKED SINCE AUGUST 31, 2001. DEGREE OF DISABILITY HAS NOT BEEN DETERMINED AT TH IS TIME.

MAINTENANCE MAN WAS CALLED TO MUDROOM TO ADJUST THE TRACKING OF THE CONVEYER BELT UNDER THE MIXER. NO ONE WAS AWARE THAT HE BEGAN TO ADJUST THE BELT. HIS RIGHT HAND/ARM BECAME ENTANGLED IN THE TAIL ROLLER. TWO MEN RESPONDED TO HIS SCREAMING AND FOUND EE WITH HIS RIGHT ARM IN THE TAIL ROLLER UP TO HIS SHOULDER. RESULTED IN BREAKING HIS RIGHT UPPER ARM, IT PIERCED HIS SKIN.
