Employee was at the breaker on the receiving end. Employee was turning around a stone to be processed into small pieces. Employee admitted to being distracted/looked away, the tip of employee's right index finger was smashed by the shifting stone. Employee was wearing gloves, employee was sent to the ER to evaluate. Awaiting results from follow up.
Employee used improper procedure when operating breaker which caused finger to be pinched in between metal table and rock. Right pinky finger was crushed.
Checking oil leak on bobcat 95xtd, malfunction of equipment caused it to roll forward and strike rt. foot and ankle of employee causing possible fracture. Contacted case tractor and filing report, safety feature of equipment malfunctioned cuaisng this incident.
EE MASHED FINGER WHILE HANDLING STONE. RECEIVED 6 STITCHES IN FINGER.
EE REPORTED PAIN IN LOWER BACK GOING DOWN BACK OF LEG-SENT TO ER AND SPECIALS EE HAS HERAIATED DISK, UNDERGOING TREATMENT AND P.T. AT THIS TIME.
SLIPPED WHILE DISMOUNTING BOBCATE, HIT FORK CARRIAGE CORNER W/UPPER LEFT INNER THIGH CAUSING PUNCTURE WOUND.
