B & T Material LLC · Struck by falling object

Miner was repairing broken chain link on drill. A wooden block was placed under the broken area to support the weight of the chain. The drill operator boomed the mast back up to change the link safely on the deck. Forgot to secure the wooden block. The block fell out from 8 feet up, striking the employee in the top of head, resulting in 9 staples.
