Accident type, without injuries

Front end loader was screening riprap and a water truck drove in behind the loader to wet material. The loader backed up and hit the front of water truck, there were no injuries to any employees (only 2 employees working) only damage to the front of the water truck.

While miner was climbing up onto the grizzly grid to clear it free of rocks, miner put a hand on the grid to push themselves up, miner's elbow pushed into ribcage and possibly bruised them or cracked them.

Employee was un-tiring the safety bar on the grizzly that keeps the grid up for us to work, when the safety bar came down on his finger, cutting his left index finger, was wearing gloves at the time.

EE had received his audiometric test and the result were abnormal in his right ear. The physician recommended that the miner should receive another audiometic test and re-evaluate both result to determine the correct action to take. As of 12/27/2010 the employee was notified.

Miner was exiting the D7 Dozer he was operating, from the leftside of equipment. The Miner proceeded to step down from Dozer, when his left knee twisted. On 01/29/08, EE injured his left knee, but EE did not request any time off. At this time no lost time injury was reported, but on 2/19/08 at the request of his doctor, EE did not report to work.

EMPLOYEE WAS ATTEMPTING TO REMOVE A RUSTED NUT FROM A BOLT WITH A RACHET & SOCKET, WHEN THE SOCKET SLIPPED OFF CAUSING HIM TO FALL BACK. WHEN FALLING BACK, HE PLANTED HIS LEFT FOOT TO AVOID FALLING ON LOOSE DIRT & GRAVEL CAUSING HIM TO TWIST HIS ANKLE & BREAKING HIS ANKLE.
