Employee was attempting to cut a small diameter hose with a grinder. The hose was coiled and when the employee cut through the hose it un-coiled and forced the grinder into their thumb causing a laceration that required approx. 5 stitches

Employee was cleaning out from under the end gate. The gate slipped off of the prying tool and cut off employee's ring finger

Employee was attempting to loosen a lock nut on an adjustment bolt. The nut broke loose and the employee fell and twisted left knee.
