Employee was on 3rd step of ladder inspecting a sand crew. As he stepped down he missed the 2nd step on ladder falling backwards onto his left arm and shoulder.

Non EE came to the pit to pick up vehicle he had purchased from the company.He was suppose to load and haul the vehicle away. While EE went to obtain a chain for the Non EE to use, he attempted to cut the vehicle with a torch. The vehicle caught on fire burning Non EE on approx. 80% of his body.
