Employee was getting ready to operate plant. His right arm was caught between the bearing and tail pulley. Right arm amputated and face laceration.
While cleaning along side screening plant, a particle of debris had gotten in his eye. I flushed impacted eye, numerous times. The employee still felt pain. I then sent him to our medical center for futher treatment.
Lifting oxygen cylinder to secure to rack on service truck, felt pain stomach area.
