Employee was removing a support brace on a shaft mounted gear box/ motion assembly without properly securing it from falling.

CLIMBING DOWN LADDER FROM CATWALK. SLIPPED AND FELL TO GROUND LANDING ON HIS BACK. CATWALK IS LOCATED ON WASH PLANT.

SLIPPED ON LADDER WHILE EXITING LOADER. JAMMED LEFT HAND ON LADDER STEP. STRAINED LEFT HAND.

SCALPING SCREEN CHUTE PLUGGED. EMPLOYEE STOMPED ON A ROCK WITH HIS RIGHT FOOT. FOOT BROKE.
