Phoenix San-Man · Struck by flying object

Employee was welding and when he completed his welding operation he raised his visor and turned his head and the wind blew dust into his right eye. He went to Concentra and was treated with a wet cotton swab that removed the dirt. The doctors also treated him for Other Mucopurulent Conjunctivitis.

EE was walking across the yard to get into his loader and as he was walking across the yard he was struck by a pickup truck going the wrong way at too fast of a speed. The driver, who works for Maricopa Redi Mix, hit EE with his personal pickup truck and EE went flying through the air.

Employee was exiting from a platform of a 980 H loader. When he stepped off of the platform with his right foot, his right foot slipped of the rung and he sprained his right ankle. The rungs of the loader are a different configuration then the older type 980's; the rungs are set further in and this is what caused the accident.

Twisted back while dismantling and moving crusher equipment. Alledgedly happened on 7/15 but employee worked 7/16 & 7/17 then called off work on Mon. 7/19 and said he was going to the chiropractor. He was observed moving gingerly on 7/16 and when plant manager asked what happened he said he thought he may have twisted his back on 7/15 but that he would be ok.
