Miner reached into tail pulley of the overland conveyor belt to remove debris on belt at which time the belt was started and pulled miners right arm into pulley causing the loss of right arm. There was 3 other miners working with victim, they got individual away from conveyor called 911 and got EE stabilized.

EMPLOYEE WAS WORKING INSIDE OF A TRACKHOE BUCKET WHEN HE FELL OUT ONTO THE GROUND. HE WAS ATTEMPTING TO CLEAN THE FEEDER.

THE EMPLOYEE WAS SHOVELING A DITCH AT THE WET PLANT.
