Phoenix San-Man · Caught in, under or between a moving and a stationary object

EE lifted tailgate to let rocks fall so gate would close completely and when lifting the gate back the third time, did not let go of gate quick enough and gate came down and smashed 2 fingers. EE was taken to Arrowhead Hospital immediatly and treated there and at John C Lincoln.

Employee was installing a roller on the under belt of the cone crusher when he twisted wrong and hurt his back. He didn't report this until Monday July 30, 2007. He came to work on Tuesday July 31 but left early to go to the clinic. They put him on light duty so he came back to work on Aug. 2. He then went to his own doctor and he told him to take two weeks off.
