Fall to the walkway or working surface

EE raised trailer bed and was attempting to loosen stuck material with a shovel. EE slipped and fell to the gound. EE sustained skull fracture/back strain. EE was treated and released from emergency room. Appointment for follow up is scheduled.

After high winds knocked over one 36""X60' conveyor onto another conveyor it had to be lifted up and standing legs removed in order to replace it with a new conveyor. While the standing legs were being dismounted off of the fallen conveyor the employee was bumped with the legs.

While changing cutting edge on loader holding bolt in place - the bolt wasn't all the way in and the impact wrench made it slip and slice my left index finger.

Fall from machine

While adjusting crusher curtains employee had to unhook his safety harness to move to one side. When he stepped to one downward he lost his balance and fell to the catwalk deck. Fractured Left Clavicle.
