Over-exertion (Not Elsewhere Classified)

While performing work on a slope, employee was repositioning filter fabric. Upon completion, the employee turned to descend the slope and, during the initial step, experienced a right ankle roll due to an obstruction beneath the fabric.

Caught in, under or between a moving and a stationary object

An employee was manually adjusting the forks of a loader with assistance from the client. The employee was supporting the tip of the fork, while the client's employee was lifting and moving the fork from near the backstop. During this movement, the fork dropped, and the employee pinched their finger between the fork and the ground.

Equipment operator was driving an open cab skid steer loader when dust blew into eyes. The operator was wearing PPE at the time. Operator sought independent medical treatment and was given subjective restrictions.

Miner was walking on a slope during survey activities when miner slipped and fell on back.

No known cause or condition. The body of a deceased trespasser, non-miner was discovered on 4/1/15 @ approximately 1:05 PM on the upper road to weasels butte within the proposed project area.

Pushing rod into pipe truck.

Tube was latched onto and pulled out of the hole. Safety pin was applied to the overshot before hoisting core tube out of hole. As driller and helper began lowering tube one of the two lifting dog teeth broke off completely allowing tube to fall to the deck. The core tube landed on an block on the drill deck, which caused the tube to kick up and strike helper in left eye brow.

Driving a ""T"" post into the ground with a 12lb pounder, the pounder came off the post smashing the left thumb between post and the pounder.
